Client Disclaimer
Question (2010): I have my first patient on Friday for a smoking cessation session, and upon talking to a friend of mine who recently qualified as a hypnotherapist (not LCH) she asked if I had sorted out my patient agreement forms.I believe the gist of it is a form whereby the patient signs to accept that he/she enters treatment of their own free will and sounds very similar to a disclaimer. It was also suggested that a written agreement for the one month aftercare after smoking cessation treatment would be a good idea.
This all seems very formal to me, and I’d have thought that having insurance would negate the need for a disclaimer?
