Confidentiality Waiver

If you are asked to provide confirmation that a person has or is attending for treatment; or requested to provide information about the symptoms or treatment of a patient to a third party, you will need to obtain written permission from the patient before so doing. A draft form is provided here, so that you can simply print out and give to your patient.

If you are required or asked to provide information about a patient’s treatment, even if only to confirm that the individual has attended, it is necessary to obtain the express written permission from that individual.
