There has long been much discussion amongst hypnotherapists in general about consent forms and how much information can or should be gained through them.
Whatever other purpose you may have for such a form, and whatever information you request from your patients, it is necessary that your patients explicitly give their consent to your privacy policy.
Therefore some kind of form is necessary for all practitioners.
