Lack of Co-operation

A patient fails to respond to any and all techniques and seems to not wish to lose her problem. What might be the reasons for this? How might I handle the situation to enable her to improve her life? What sort of questions should I be asking to overcome this situation?

The following is an email I received from a concerned therapist about a patient who had not co-operated in her treatment and thus had not gone along with any of the various techniques introduced.

The treatment was terminated but the therapist wished to learn something from the experience, wanting to understand the possible reasons for the lack of co-operation (was it the patient not wishing to succeed, lack of clear guidance from the therapist, a symptom of the issue she had presented with) and to know if (and how) it may be possible to prevent a similar situation occurring in the future.
