Question: Re patients’ records cards, should I try to keep as few notes as possible? Is there some way of deciding which details are worth noting and which not?
To answer your second question first; if you feel that information might or could be of use, then keep a note of it. It is better to have something that turns out not to be needed than to be missing details that you do require. As for the amount of notes you keep; this is entirely down to individual requirements and preferences. Certainly, on the record card you use during the treatment session, I would recommend
